Part-time Geriatric NP needed in NY

We become an extension of the family to expertly coordinate care at home that we would want for our own parents. We launched in New York this year and are met with surging demand. Looking to bring on additional clinical capacity to meet the patient demand.

Part-time, fully remote opportunity:
We currently have an opening for a NP for up to 5 hours a week to facilitate telehealth visits with older adults and their families. The primary responsibility will be a health assessment confirming they have a principal illness which will qualify them for Medicare-covered care management services. Long-term opportunity to transition to full time if interested.

We are looking for a NY licensed NP who is experienced with working with older adults and is interested in supplemental work. We are looking for someone who can quickly establish trusting relationships with families as the first face they see from Hera.
