We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive primary care to a geriatric population through home care, nursing home visits, and outpatient clinic coverage in Amityville, NY. This role offers a manageable patient panel, allowing for high-quality, patient-centered care. Compensation & Benefits:Base Salary: $160,000 - $170,000 plus annual bonus. Full benefits package including health insurance, retirement plans, CME allowance, and PTO. Position Highlights: Full-time schedule Monday - Friday – No nights or weekends. Primarily home care/nursing home visits with some outpatient coverage in Amityville. Small patient panels for personalized care (4-5 patients per day in the field, 7-10 per day in the clinic). Comprehensive primary care with a focus on continuity and proactive management.  Requirements: Licensed Nurse Practitioner or Physician Assistant in New York. Minimum of 2 years of primary care experience required. Experience or strong interest in geriatrics preferred. Excellent clinical, communication, and organizational skills.
