Pediatric Integrated Behavioral Health Consultant

The Pediatric BHC provides assessment/diagnosis, crisis intervention, risk assessment, and short-term therapy to primary care patients. The BHC collaborates closely with the Physicians, their medical team, and external referral resources to ensure that patients receive the appropriate level of behavioral health care. The BHC navigates patients and families through the behavioral health landscape in an effort to improve clinical outcomes and mitigate patient cost. Coverage includes a combination of both in the moment responses to Physician and patient needs, telephonic/virtual consultation, as well as scheduled patients. The BHC provides staff training around population specific behavioral health needs within an integrated practice framework. The BHC must be highly organized and energetic with a great attention to detail, adaptable to frequent change, and able to work autonomously in clinical decision making.
Responsibilities:
Utilizing clinical techniques and theory, the BHC consults on, assesses, and intervenes with patients and their families regarding emotional and social problems. Duties include conducting assessments, determining diagnosis, planning and implementing treatment plans, evaluation, education, and coaching of the patient’s family members. The position consults, collaborates, and communicates with a wide range of social services, school professionals, and outpatient referral resources to meet patient and family needs.
· Assesses patient and family to ascertain emotional and social problems
· Identifies those issues which must be addressed during the patient's care, and formulates a treatment plan in collaboration with the patient and family
· Provides timely referral resources for patients and their family and follows up telephonically, via email, or in -person to confirm successful access to care
· Consults and collaborates with the Physician and team members to ensure that patient's emotional and social needs are addressed
· Provides brief therapeutic intervention and/or psychosocial support to the patient and family
· Provides Physician support in the form of standardized appointment follow ups surrounding behavioral health needs including but not limited to psychopharmacology
· Documents each patient encounter in the electronic medical record and keeps the Physician apprised of progress or concerns
· Assists in the operation of an integrated behavioral-medical model by participating in Physician or supervisory meetings to establish practice protocols and facilitate staff trainings
· Identifies weaknesses in behavioral health work flow and recommends improvements

Knowledge and Skills:
· Knowledge of community resources and agencies that serve the children and families
· Advanced interpersonal skills to interact effectively with practice staff
· Must be comfortable in a fast-paced work environment
· Ability to work in an interdisciplinary team with a high degree of communication
· Computer literacy with ability to use standard word processing, Outlook services, and EMR’s
· Experience and comfort with use of standardized behavioral measures as a tool in clinical decision making
· Experience with brief intervention models including Solutions-Focused, Motivational Interviewing, and Cognitive Behavioral Therapy
· Knowledge and experience treating commonly recurring pediatric behavioral health concerns
· Basic knowledge of psychopharmacology
