Flexible In-Home Health Evaluator – NP or PA
California – Multiple Counties | Home Health & Primary Care | Per Diem to Full-Time
Join a nationwide healthcare network delivering compassionate, in-home health evaluations for Medicare Advantage and Medicaid members. We're seeking experienced Nurse Practitioners or Physician Assistants in California to conduct pre-scheduled, in-home visits in various counties. Position Overview:
• Specialties: Geriatrics, Home Health, Hospice & Palliative, Internal Medicine, Med-Surg, Primary Care, Public Health, Urgent Care
• Start Date: As soon as credentialed
• Contract Type: Short-term or long-term options available
○ Full-time, part-time, contract, PRN, travel, seasonal
• Schedule: Fully flexible — choose your days (Monday–Sunday, 7 AM–7 PM)
• No nights or weekends required

Job Details:
• 6–9 in-home visits/day
• Conduct health risk assessments — no prescribing, no labs, no follow-ups
• Pre-filled forms provided for each patient
• Medication review, physical evaluation, and medical history review
• Serve Medicare Advantage, Medicaid, and select commercial members
• EMR: TBD

Requirements:
• Board Certified NP or PA
• Active California license
• Master’s degree required
• Reliable transportation and ability to travel within assigned region
• Strong communication and independent clinical decision-making skills
