Job description

A growing healthcare provider focused on delivering compassionate, patient-centered care in the home setting is seeking a dedicated Nurse Practitioner (NP) to provide in-home primary care services to their geriatric patient population across the Greater Philadelphia area. As a Nurse Practitioner the candidate will conduct home visits to provide primary care assessments, follow-ups, and preventive care for elderly patients. This is a field-based role, not clinic-based, and requires travel to patient homes throughout the region.
