Qualifications / Requirements:
• A minimum of 2 years’ experience as a family-practice primary care provider recommended
• Preventative care and health coaching experience also required
• Occupational health experience is a plus
• Excellent computer skills required including familiarity with Microsoft Office and electronic health records
• Excellent interpersonal skills

Licensure Requirements:
Nurse Practitioner
• RN Licensure issued by the state of practice
• ANCC or AANP certification required
• DEA licensure
• Basic Life Support (BLS) Certification

Physician Assistant
• PA Licensure issued by the state of practice
• NCCPA certification required
• DEA licensure
• Delegation agreement with supervising physician
• Basic Life Support (BLS) Certification

• 1 day of coverage
• WI license required
• BLS req'd
