Job Description:
Community Health Center in Ellenville Seeking FT FP
• Physician Only
• Must be BC in Family Medicine
• 6 month assignment
• M-F
• 8a-5:30p (no weekends or nights)
• 3 Days a week possible, but must be a "Rockstar" candidate
• Truly seeing ALL ages (cradle to grave)
• Basic primary care
• Must be comfortable in CHC space and dealing with underserved population
• Full support staff
• EMR: EPIC
• Must have NY License, DEA, BCLS, and Medicare/Medicaid
• Average credentialing timeline 30-60 days

• Board certified in family med
• 6 month assignment starting October
• Basic primary care seeing newborns to geriatrics
