Specialty Areas: Geriatrics, Home Health, Hospice & Palliative Medicine, Internal Medicine, Medical-Surgical, Primary Care, Public Health, Urgent Care

Start Date: ASAP (once credentialed)
Contract Length: Short-Term or Long-Term (Full-Time, Part-Time, Contract, PRN, Travel, Temporary, Seasonal)
Schedule: Flexible, Monday–Sunday, 7:00 AM – 7:00 PM (no nights/weekends required)

Position Details:

Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members

Visits include medication review, medical history, and physical evaluation; no prescribing, lab orders, or changes to treatment plans

Average visits: 6–9 per day

EMR: My Signify

Home Health setting; travel support available (airfare, hotel, rental car, $40/day for gas depending on eligibility)

Multiple County Options

Requirements:

Board Certified NP/PA

Master’s degree required

Active state license required

Provider must indicate county of interest when applying

Credentialing Timeframe: 7 days to 2–3 weeks
