NP PA position:
Cambridge City, IN
10-15 patients a day, all ages, basic primary care
DOT cert a plus but not required

MD DO position:
Ft. Wayne, IN
5 days a week Calhoun Street and Oxford location - going between the two
· 2.5 patients per hour, Must see all ages
· E-Clinical Works- EMR
· BC in any primary care specialty
· Monday/Wednesday – 8-7 – Tues/Thurs/Friday – 8-5
· Cred- 30-45 days
· DEA, CPR, CSR- required
· Support staff- Need to confirm if each provider is given a MA
· T&L- Hotel + Mileage
