Job Description:
• PC APRN
• Primarily Joliet facility, may need to go between there and Bolingbrook
• ASAP start following 30-day credentialing
• 6-month assignment
• 40 hours/week (highly prefer M-F 8-5 but would look at 4 10’s)
• Every 4 weeks schedule will be Tues-Sat
• Suboxone preferred for medication assisted treatment program
• BLS, ACLS
• 14-20 PPD
• All ages
• RN’s, MA’s for support
• NextGen EMR
• Initial phone screen with CMO then in-person interview

• 14-20 ppd
• Basic primary care - all ages
• Mon-Fri 8-5
