Job description

Aya Locums is seeking Nurse Practitioners and Physician Assistants for flexible contract opportunities in Southwest Louisiana. This assignment offers a wide range of practice areas, including geriatrics, home health, hospice & palliative care, primary care, urgent care, and internal medicine. Providers will conduct pre-scheduled, in-home health evaluations for health plan members, with no prescribing or treatment management required.

Assignment Details
• Start: As soon as credentialed
• Contract length: 3 months with the option to extend
• Schedule: Flexible, Monday–Sunday, 7a–7p (no nights or weekends required)
• Average daily visits: 6–9

Case Mix / Work Details
• Geriatrics, Home Health, Hospice & Palliative Care, Internal Medicine, Medical-Surgical, Primary Care, Public Health, Urgent Care
• Department description: Conduct in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
• Visit components: Medication and medical history review, physical evaluation
• Notes: No prescribing, ordering labs, or altering treatment regimens
• EMR: TBD

Practice Model
• Home health, field-based setting
• Independent patient evaluations with administrative and travel support

Requirements
• Board Certification: Required
• State License: Required (Louisiana)
• Education: Master’s degree required
