Location: Multiple Sites in San Jose, CA
Start Date: As soon as credentialed
Contract Length: 6 months

Schedule

Monday – Friday, 8:00 AM – 5:00 PM (lunch required)

8-hour shifts

Full-time coverage needed

Practice Details

Setting: Outpatient Primary Care

Patient Population: All ages (must be comfortable treating both children and adults)

Average Patient Volume: 21 patients per day

Support Staff: On-site clinical and administrative support

EMR: Epic

Requirements

Board Certification: Required (NP or PA)

State License: Active and unrestricted (required)

Certifications: BLS

Experience: Prior primary care or family medicine experience preferred
