Family Medicine APP Needed in San Jose, CA

26 week assignment

Locations: Float between clinics

BACH's needs change day to day. Please ask your provider what they are willing to work. BACH has Brick and Mortar clinics, School Based Clinics, and Mobile Clinics. During the interview all of these will be brought up unless you and your provider are very clear on what they are willing to work on the front end.

This is a Brick and Mortar (Buildings) Clinic float pool. Providers will have to float between locations. Some are close in distance while others are roughyly 30 minutes to an hour away.

Providers will be working with ALL AGES, MUST be comfortable working with kids and adults.

RATES ARE ALL-INCLUSIVE

Three Different Locations! Please present with the providers preference, if they are able to go to all three they will be more likely to get an offer.

Shift Details: 8 hour shifts
Schedule: Mon – Fri 8am – 5pm (lunch required)

• EMR: Epic

Practice Setting:
• Outpatient Clinic

Requirements:
• Board Certification: required
• State license required: required
• Certifications required: BLS

Credentialing Timeframe: 30 days or less

ORIENTATION IS A FULL WEEK, MONDAY - FRIDAY! PROVIDERS MUST BE ABLE TO ATTEND ALL OF OREINTATION IN ORDER TO PRESENT
