Job description

Provide comprehensive primary care services to adult patients in an outpatient setting.

Manage chronic conditions and preventive care consistent with HMO and quality performance measures (e.g., HEDIS, annual wellness visits, medication adherence).

Perform patient evaluations, follow-ups, and care coordination within a multi-disciplinary team.

Ensure documentation accuracy and compliance with HMO and CMS requirements.

Collaborate with clinic staff and IPA/medical group partners to optimize patient outcomes and continuity of care.
