Primary Care Physician Needed for In-Home Health Evaluations in Brooklyn, NY | Flexible Schedule | No Call | New Grads Welcome

Job description

Start: September 10, 2026 (or as soon as credentialed)
Length: 13 weeks (open to extension)
Coverage: In-home health evaluations, scheduled
Schedule: Flexible Monday-Sunday, 7:00-19:00 (5x8s template); no nights or weekends required
Call: No call
Volume: Average 6-9 visits per day
Practice Setting: Home health - evaluations conducted in the member's home

Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Review medications and medical history and complete a physical evaluation in the member's home. Document on a pre-populated, patient-specific form. No prescribing, lab ordering, or alteration of treatment plans. Orientation modules of approximately 10-12 hours required prior to start (unpaid outside of California).

Requirements
Board Certified in Primary Care
Active New York license required, must be in hand (not open to licensure)
DEA not required - no prescribing or diagnosing
No COVID-19 or flu vaccination requirement
New graduates accepted
7:00-19:00 flexible day scheduling, Monday-Sunday
No nights or weekends required
No call
