Schedule: Mon – Fri 7A – 3P, 6/23 – 9/19
Work Details:
· Patient population/age: adult, pediatric, neonatal
· Providers Requested: 1
· Trauma Level: 4
· EMR: Paper Charting
· Skills required: general, adult, pediatric, neonatal, trauma, spinal, epidural, regional, obstetrical, neurosurgical, thoracic, complex ortho, post-op epidural, post-op extremity blocks, Use of x-ray guidance in performance of blocks, Ventilation management, Evaluation and management of acute volume/BP issues

Practice Setting:
• Inpatient/Outpatient

Requirements:
· Board Certification: Yes
· License required: Active AK License
· Certifications required: ACLS
· Credentialing Timeframe: 45 days

• Airport name: Anchorage
• Holiday and OT - max 1.3x the Regular and Oncall Rates
• Travel Caps:
o Car Rentals - $45 per day
o Airfare - Roundtrip $800
o Lodging - Anchorage - $220/night
