CRNA Needed in Phoenix, AZ

Job description

12 hour shifts-Supervised Model
Must be flexible on shifts (days, nights, weekends(at least 1 per month)) Willing to do all nights.
Can offer 3-4 12 hr shifts/week - Minimum of 4 weeks ideally looking for 36 or more hours a week per month, night, weekend and day
Flexible locums can work as many as they want
Must be willing to do OB
Level 1 Trauma
No travel covered, no billable modifiers for OT/Holiday (rate is flat)
AZ licensed = 6 weeks
IMLC= 2 months
Non Licensed= 3 months
