ASAP – 3 Months with option to extend
◊ Shift Schedule Details & Shift Duration: Multiple Shifts – 7a-5p, 9a-7p, 11a-9p (Other shifts available) - Wednesday - Saturday or Thursday - Sunday. 4x10's. Flexibility is helpful but will be consistent with the schedule - greatest need is the 7a-5p.
◊ Is call required? No Call
◊ # of ORs currently being run: 5 Different Locations in Fontana/Ontario – 3 Locations in Fontana – 2 Ontario – 2 Open Saturday/Sunday’s - Would be in main hospital based in the OR's either Fontana or Ontario.
◊ Type of Anes Model and supervision ratio – Care Team Model – 1:3 Anesthesiologist/CRNA Ratio – 70 Anesthesia providers on staff
◊ Case types: General Surgery, Orthopedics, GYN, Ophthalmology, Urology, Neuro, Healthy Pediatrics (Ages 1-18), Thoracic, possibly OB L&D (May consider providers that can’t do Thoracic, Neuro, L&D). All Peds under 1 year old cases handled by Pediatric Anesthesiologist

• 4x10's, 7a-5p days & or 11a-9p afternoons, Weds-Sat or Thur-Sun rotations, all cases see PD, labor & del not required or thoracic, 5 different sites between Fontan & Ontario
