Type of cases required: BOTH SITES: PEDs, General, Ortho, URO, Ophthalmology, ENT, Plastics, Podiatry / Add'l Cases _ Rock Island: CARDIAC, ENDO and VAS /Add'l cases _ Moline: GYN and Pain Mgmt

Supervision: Medical Supervised Model

Spinals, Blocks, Epidurals - must be proficient: Yes **MUST BE PROFICIENT IN BLOCKS**

Credentialing Timeframe: 90 Days

Caselogs required: Yes - proof of epidurals, spinals and blocks

EMR System: Epic

Vaccination Required: Need to be vaccinated / accepting religious / medical exemptions

Licenses: ACLS, BLS, PALS
