Job description

Monday–Friday, 7:00 AM–3:30 PM
Hours may extend with add-on cases
First-day arrival: 6:30 AM for orientation
Call: Possible 1 weekday; weekend call 1:7 (both not required)

General
Cardiac assist (MDA primary)
Thoracic & Vascular
OB
ENT
Ophthalmology
Ortho
Urology
Pediatrics (healthy; mostly ENT)

Requirements
Active OH license
Board Certified CRNA
OH Medicaid (KY Medicaid if applicable)
CAQH & NPI access
Procedure logs
Drug screen, immunizations, fit test
Credentialing: ~2 weeks with fast submission
EMR: Meditech & SIS

Compensation
Standard: $220/hr + T&L
Holiday (Christmas/New Year): $240/hr + T&L
