## 🩺 CASE MIX:

* ENT
* General surgery
* Orthopedics
* Neurosurgery
* Gynecologic surgery
* Low-risk obstetrics
* Urology
* Minimal healthy pediatric cases
* Regional blocks

## 💰 COMPENSATION — 1099:

* **Base Rate:** $190/hour
* **Regional Blocks Stipend:** +$5/hour
* **Pediatrics Stipend:** +$5/hour
* **Total Base Rate with Blocks & Pediatrics:** **$200/hour**
* **Full-Time Estimated Compensation:** $325,000–$345,000 at 1,712 hours

## 💰 COMPENSATION — FULL-TIME W2:

* **Base Compensation:** $238,000–$260,000 based on experience
* OB Stipend: $5,000
* Pediatrics Stipend: $5,000
* Regional Blocks Stipend: $5,000
* 403(b) match beginning Year 2
* 8 weeks / 320 hours vacation per 1.0 FTE
* Dues, subscriptions, CME & board certification: $4,000
* Signing bonus / student loan support: Up to $100,000 over 5 years
* Paid holidays + birthday pay
* Overtime / Call-Back: $200/hour
* Additional on-call compensation available
* Vacation payout: 120 hours at hourly rate
* Medical, dental, vision, life insurance & malpractice coverage

## 📞 CALL:

* **Night Call — Findlay & Bluffton:** $250/night
* **Weekend Day Call — Findlay:** $1,200/day
* **Weekend Day Call — Bluffton:** $800/day
* Equitable call schedule

## 🛑 SUBMISSION REQUIREMENTS:

* **Board Certification:** Board Certified CRNA — Required
* **Ohio License:** Active OH license or willingness to obtain
* **DEA:** Must be obtained prior to start; no exemptions
* **NPDB:** Most recent Self-Query Report pulled within 30 days — required at presentation
* **Malpractice & License:** Clean history required
* Must specify:

* Part-time or full-time preference
* W2 or 1099 preference
* Preferred schedule option
* Date of agency's last communication with provider
* Reason for interest in the opportunity
* Visa support requirement, if applicable
