Contract Length: 3 months with option for extension
Schedule: 7:00am – 7:00pm
Support Staff: CRNAs
Patients per day: 50-60

Arizona License Required (We can help license you)
Credentialing Timeframe: 30 days
Medical malpractice insurance, travel, and lodging provided
