Schedule & Coverage
• Scheduled + Call required
• Start: November/December 2025
• Hours: Mon–Fri, 7:00a–3:00p (8 hrs)
• Call: Secondary call, from home, 3:00p–7:00a at least once per week
• Call response time: 30 minutes
• Weekend call: None
• Dates needed: Sept 24–25, Dec 15–19, Dec 21–24, Dec 26, Dec 29–Jan 2, Jan 13–16, Jan 18
Details
• Reason: MLOA coverage
• No supervision – CRNAs work independently
• Cases: General, Ortho (mostly spine), Urology, Ophthalmology, GYN, limited Dental/Peds Dental, ENT, Podiatry, heavy OB
• Must be proficient in spinals, blocks, epidurals
• Case logs required (spinals/epidurals/blocks)
• Providers: 1–2 MDs / 6 CRNAs
• 6 ORs, 2 Endo, 1 Cath Lab, 1 IR Room
• EMR: EPIC
Requirements
• License: IN (IMLC accepted)
• Certs: ACLS, BLS, PALS, Board Certified (not BE)
• Credentialing: 30–60 days (temps available with clean history)
• Name clear required (Full name, specialty, NPI, site)
