SPECIALTY CARDIOLOGIST (GENERAL / INTERVENTIONAL)
PATIENT AGE GROUP 18 yrs and older
SPECIAL PROCEDURES/DETAILS Cardioversion, temporary pacemaker placement, coronary & peripheral interventions, cardiac diagnostic testing interpretations
CERTIFICATIONS REQ’D/PREFERRED? BLS / ACLS
BOARD CERT REQ’D/PREFERRED? BC required
STATE LICENSE REQ’D/IMLC? Indiana
DEA REQ’D? Yes
CSR REQ’D? Yes
WORK EXPERIENCE REQ’D? Minimum of 5 years

START DATE 7/14/25
CONTRACT LENGTH 6-9 months
COVERAGE TYPE 40 hr week, no STEMI call after hours. 1:2 call (patient calls only)
# OF SHIFTS & HOUR 4.5 days a week or 9 out of 10 days
SCHEDULE DETAILS 2-3 full days in office & 2 ½ days in cath lab
