Case Manager - Baltimore, MD
Job Type: Full-time (On-site)
Schedule: Monday-Friday with some flexibility; rare weekend work under special circumstances
Travel: Required for home visits; must have reliable transportation and a valid driver's license
Responsibilities:
Manage a caseload of high-risk clients, from intake to discharge.
Develop and implement individualized care plans.
Provide counseling, therapy, and home/virtual visits.
Coordinate access to community resources and services.
Maintain accurate records in the Penelope case management system.
Ensure compliance with policies, funder requirements, and regulations.
Audit client files for quality and documentation accuracy.
Participate in program planning and quality improvement.
Qualifications:
3-5 years of case management experience;
1-2 years in a lead or supervisory role preferred.
Master's degree in Social Work, Psychology, Human Services, or a related field.
Must have LMSW or LCSW or LICSW or LCSW-C in the state of Maryland
Knowledge of local community resources and support systems.
Proficiency in case management software and EHR systems.
Bilingual skills preferred but not required.
Working Conditions:
Combination of office-based, virtual, and fieldwork responsibilities.
Occasional evening or weekend hours as required.
Reliable transportation and a valid driver's license are mandatory
