Licensed Master Social Worker – Nursing Home Surveyor/Complaint Investigator

✅ Key Responsibilities
✔️ Investigate nursing home complaints, including abuse/neglect
✔️ Conduct surveys to ensure regulatory compliance
✔️ Draft detailed reports & Statements of Deficiencies (SOD)
✔️ Verify Plan of Correction (POC) compliance
✔️ Enter findings in Federal/State databases
✔️ Attend hearings and DOH meetings as needed

✅ Qualifications
✔️ Master’s in Social Work (MSW)
✔️ NYS Licensed Master Social Worker (LMSW)
✔️ 2+ years clinical experience (nursing home preferred)
✔️ Regulatory/survey experience highly preferred
✔️ Strong documentation, analytical & communication skills
✔️ SMQT certification a plus
