Licensed Master Social Worker – Nursing Home Surveyor/Complaint Investigator

Greenlife Healthcare Staffing is seeking a Licensed Master Social Worker (LMSW) – Nursing Home Surveyor/Complaint Investigator in Syracuse, NY. This position provides an opportunity to use your clinical and investigative expertise to support quality of care and regulatory compliance in long-term care facilities.

Qualifications
Master’s degree in Social Work (MSW) from an accredited program
Current New York State LMSW license
Minimum 2 years of clinical experience in a medical facility; nursing home experience preferred
Two years of nursing home surveillance experience preferred
Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months
Valid driver’s license and ability to travel to onsite facilities
Strong investigative, analytical, interpersonal, written, and verbal communication skills
Ability to interview residents, families, facility staff, and other healthcare professionals
Ability to work independently with minimal supervision and effectively within an interdisciplinary team
Strong computer skills and ability to learn new systems
Flexible schedule and availability for rotating weekend, holiday, and evening assignments

Responsibilities
Participate in nursing home surveys, complaint investigations, and surveillance activities involving allegations of abuse and neglect
Evaluate compliance with Federal and New York State requirements
Conduct interviews and review facility practices, records, and services
Prepare investigative reports and Statements of Deficiencies (SODs) within required timeframes using Principles of Documentation
Assess compliance with Plans of Correction
Participate in emergency surveys at Skilled Nursing Facilities (SNFs)
Participate in State monitoring during Immediate Jeopardy situations
Complete required data entry in Federal and State reporting databases
Attend required meetings and testify at administrative hearings when necessary
Follow Department of Health (DOH) and IPRO schedules, policies, and procedures
Travel to assigned onsite locations according to the work schedule

Compensation & Benefits
$39/hour
Full-time, temporary-to-permanent opportunity
Premium pay for holidays and overtime
Mileage/travel requirements associated with onsite assignments
5 sick days (40 hours), subject to the NYS Paid Sick Leave Act
Health insurance, subject to plan eligibility
Specialized experience in healthcare regulation, compliance, investigation, and long-term care quality
