Requirements
✓ NYS Licensed Master Social Worker (LMSW)
✓ Minimum 2 years of clinical experience in a medical facility (nursing home preferred)
✓ Experience conducting nursing home surveillance activities (preferred)
✓ SMQT Certification preferred (must be obtained within 12 months if not currently held)
✓ Ability to travel to on-site locations
✓ Strong documentation and investigative skills

Responsibilities
✓ Conduct surveys and investigate complaints, including abuse or neglect allegations
✓ Perform regulatory surveillance tasks in long-term care facilities
✓ Draft Statements of Deficiencies (SODs) using the Principles of Documentation
✓ Assess compliance with Plans of Correction
✓ Enter data into federal and state reporting systems
✓ Participate in state monitoring during Immediate Jeopardy situations
✓ Attend required meetings and testify at hearings as needed
✓ Follow DOH schedules, policies, and procedures
