📋 Contract Details
🟢 Start Date: ASAP, as credentialing allows (provider does not need to be fully enrolled prior to start date)
🟢 Contract Length: 6 months
🟢 Contract Type: Locum Tenens
🟢 Shift/Schedule: 8-hour shifts (8:00 AM – 5:00 PM; some flexibility)
🟢 Setting: Outpatient clinic
🟢 Patient Demographics: All ages (no restrictions)
🟢 Average Patients per Day: 21–30
🟢 Required Procedures: Biopsy, excisions, cryotherapy, injections, ED&C, light therapy, patch testing
🟢 Work Model: Provider will work solo 1–2 days per week when permanent physician is onsite; will work solo or alongside another locum when perm physician is on leave
🟢 Licensure: Must have an active WA license. If IMLC, LOQ required at time of presentation.
🟢 Certifications: BLS, ACLS
🟢 EMR: Epic – proficiency required
