****must be residency trained and BC (BE if within 5 years of residency) in FP, IM/Peds or ED BY AMBS (American Board of Medical Specialties or AOB American Osteopathic Board

Please provide the following:
- at least 2 WEEKS worth of availability, the more the better.
- MUST BE WILLING TO WORK 12 HOUR SHIFTS
- Preference will be given to providers who can work blocks of shifts and weekends and weekdays.
-Can provide copies of active license to ensure clean background.

MINIMUM QUALIFICATIONS:
***Physicians: Residency training and board certified/eligible in Family Medicine (FM), Emergency Medicine (EM) or Med/Peds (this is required).
*must be residency trained and BC (BE if within 5 years of residency) in FP, IM/Peds or ED*
*MUST be trained to see both adults and pediatrics.
*Physicians who are board eligible must have completed board certification within 5 years of residency completion. *
*Board certification must be obtained during appropriate eligibility time frame.*
*Current certification or board eligible with achievement of certification within five (5) years) leading to certification in relevant specialty by the American Board of Medical Specialties or the American Osteopathic Board.*

We CANNOT accept Board Certification from other boards not ABMS or AOB
* Must treat both pediatric and adult patient populations (all ages but generally 6 months and up)
*Comfortable with minor procedures (sutures, casts, splints, etc.), reading EKGs and X-rays. We have onsite X-ray and EKG.
*Must be comfortable with EMR. Epic training will be provided but cannot be provided prior to clearing credentialing and onboarding.
Must be willing to work with medical assistants, scribes, rad techs as support staff (no RN/LPN support staff)

Scheduling/Needs:
* Up to 14 shifts per month.
Other Details: Urgent Care clinic; Provider must see patients 6 months and up for urgent care needs. Procedures including suturing, wound care, I & Ds, splinting, X-Ray and ECG.
