We are searching for a full time Post Acute Nurse Practitioner or Physician Assistant in Manchester, NH. The hours are Monday – Friday, 9AM – 5PM, no weekends no call rounding. This position involves delivering comprehensive medical care, managing chronic and acute conditions, and optimizing the well-being of patients as they transition from acute care to post-acute care.
