Requirements
✓ Bachelor’s degree in Nursing
✓ Active NYS Registered Nurse (RN) license
✓ Minimum 2 years of home care experience (CHHA preferred)
✓ Familiarity with NY State/Federal home health regulations
✓ Proficiency in Home Care Home Base (HCHB) EMR
✓ Strong communication and critical thinking skills

Responsibilities
✓ Oversee day-to-day clinical operations across assigned region
✓ Ensure high-quality care delivery and compliance with CHHA standards
✓ Support and guide nursing staff on patient care and field issues
✓ Collaborate with interdisciplinary teams and leadership on performance goals
✓ Use clinical judgment to resolve service or personnel-related concerns
✓ Participate in audits, quality improvement, and regulatory reviews
