JOB DETAILS:
Practice Setting: Free-Standing Cancer Center 
Reason for Coverage: Loss of provider
Call Schedule: No call during locum period; call expected if placed perm; call schedule is 1 week out of every 6-8 weeks
Call Ratio: No call during locum period; call expected if placed perm; call schedule is 1 week out of every 6-8 weeks
Patient Population for Required Coverage: Adult, Geriatric

What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %: 65% benign hematology, 35% malignant oncology, 0% internal medicine
Number of Physicians in Practice: 7
How Many APPs Provide Coverage: 6
Average Number of Patients Per Day - OUTPATIENT: 25
Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 20
Clinical Area: Oncology - Outpatient Setting, Oncology - Inpatient Setting
Trauma Level/Designation (Hospital): N/A
Documentation System/EMR: EPIC
Required Duties: Participation on a Tumor Board, Mgmt. of Bone Marrow Transplants, Blood Bank, Orders Tests, Orders Images, Prescribes
Administration of Chemotherapy: Nurse Oncologist
**Estimated Credentialing Timeframe: 60 Days
Facility reimbursement for the following costs: Flight, Hotel, Rental Car, Mileage Billed at the IRS Standard Rate
Submission Requirements
Board Certified in Hematology & Oncology - [REQUIRED]
Active SC license - [REQUIRED]
BLS & DEA - [REQUIRED]
Clean background required for Emergency temps - [REQUIRED]
A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
