• Schedule: Monday-Thursday: 8am-4:30pm; Friday: 8am-12pm
• Job Setting: Hospital
• Types of Cases: Electromyography (EMG), and botulinum toxin (Botox) if possible.
• Credentialing Timeframe: 90-120 days
• Board Certified
• COVID Vaccinated
• Electronic Medical Record (EMR): EPIC
