· Patient Population: All ages
· Practice Setting: Hospital
· Hospital Size: 268 beds
· Trauma Level: I
· EMR: Cerner

Requirements:
· Board Certified or Board Eligible in ENT
· Active Arizona license required
· DEA required
· Minimum 1 year experience

Credentialing:
· Emergency privileges available
