Looking for Nurse Practitioner to join our medical practice part time in Rockford, IL. We need a compassionate practitioner, who enjoys helping others, and is interested in the field of wound care. The Nurse Practitioner would round and treat patients 2-3 days per week.
Job Description:
• Patient Rounds (2-3 days per week)
• Patient exam/evaluations- Evaluates and manages patients for wound care and ensuring delivered care meets current standards of practice
• Diagnosis and Treatment plans
• Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources
• Adheres to clinical practice guidelines for wound care
• Performs care that is accurate, efficient, and consistent with the patient’s needs
• Coordinates the patient plan of care with the facility’s clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals
• Ensures that education is provided to the facility staff regarding the management of the patient’s wound(s)
• Maintains communication and a working relationship with the facility Administrator, DON, team of health care providers, and patient family members regarding wound care services
• Demonstrates competency in the position of wound care including assessment, debridement, wound dressings, skin substitutes and other treatment as needed
• Documentation- timely and accurate
Requirements:
• Nurse Practitioner Certified
• NP license in IL
• Sharps Debridement Certified
• Wound Care certified (we can assist in certification if needed)
• Previous wound care experience preferred, however experience similar field will be considered
Compensation (range):
• $65-$70/hr
